 Incidents are events that gave rise to or had the potential to lead to an accident, faults/malfunction of equipment, violations of business code of ethics and other interruptions to the operations of a business. “Root cause analysis (RCA) is one of many quality improvement approaches used to identify, understand and resolve any root causes of problems or incidents. It is basically a problem-solving technique.
